What immunizations does your learner need to enroll in MEMS?

Planning to enroll your learner in MEMS? Changes in your child's health history? Down load the Student Health History Form fill it out and return it with your learner.



Keep your learner home from school when...
Your learner has a fever of 100.0 degrees or greater
Until your student is fever free for at least 24 hours
Your learner has chicken pox and the pox are not scabbed over.
Your student is experiencieng diarrhea or vomiting (perstistent stomach ache should be evaluated by your healt care provider)
Your learner has "heavy" nasal congestion or a deep hacking cough with or without a fever
Any unexplained rash accompanied by fever (please contact the nurse)

24 hours/day, 7 days/week.

Send your learner to school when...
Your learner has a rash but there is no fever. (please call the nurse to discuss unexplained rashes.)
Your learner has the symptoms of the common cold but otherwise feels well.
Your learner has taken doctor prescribed antibiotics for at least 24 hours.

Medications At School
Medications given 2 or 3 times a day shouldb be given at home
Medications that need to be taken during school hours must be given to the nurse
A consent form must always be signed by a parent or guardian before any medication is given
All medications must be in the original container with original prescription or label
Consent forms for Tylenol are available in the health office or click the link to the left.
Do not sent medications in your child's backpack, lunch or in a zip lock bag.
